Chronic care includes populations with special health care needs who are individuals with chronic illnesses, disabilities, and older populations requiring end of life care needs (DeNisco & Barker, 2016). These populations have special needs, which are different from other populations and should receive special care services (DeNisco & Barker, 2016). These populations are also at a high risk of developing healthcare conditions and the older generations are a major concern of the health care sector due to increasing healthcare demands from this population (DeNisco & Barker, 2016). There are high costs related to end of life care, which can be reduced through healthcare insurance (DeNisco & Barker, 2016).
The number of individuals with chronic illnesses is also increasing posing a major concern for the government and the healthcare industry to cater for their needs (DeNisco & Barker, 2016). Recommendations to address the needs these populations include restructuring the healthcare industry to accommodate the aging population, population with chronic illnesses, and persons with disabilities needs (DeNisco & Barker, 2016). Strategies to manage chronic illnesses have been implemented in healthcare facilities. New healthcare systems, which are able to improve the health status of the elderly have been considered and implemented to increase the quality of life of this population (DeNisco & Barker, 2016).
Healthcare facilities have also been able to achieve this by using advanced nursing practices to manage the issues related to this population (DeNisco & Barker, 2016). The government has also enabled the older population and persons with disabilities to enjoy a longer lifespan through improved education, which directly impacts the health status of these populations (DeNisco & Barker, 2016). This has improved the overall health status and well-being of this population, which is a positive impact on the healthcare goals of the government and healthcare facilities (DeNisco & Barker, 2016). The government has also allocated 10% of its national health expenditures on long-term care for the older population, which has had a positive result (DeNisco & Barker, 2016).
